Kuhli Loach very emaciated

I have noticed that one particular Kuhli loach from my batch of eight juvenile specimens 1.5-2" in length is extremely emaciated. The others seem to be growing quite fat and larger on their diet of flake food. This one looks to be in very poor condition. His body is shrunken like a grape. Would it be wrong to bring him back to the pet shop for an exchange? I believe he is afflicted with internal parasites and I am not sure how to treat this disease without dangerously strong medications. My other more benevolent option will be to purchase a 5 gallon Hospital to recuperate this unhealthy fish.
