Bristlenose pleco death

Until tomorrow, I won't have a water test able to be done, but any ideas for now would be most appreciated.

Tank is 20gallons long. Filtered by an AC200, an AC150. One four inch air stone added in the summer to help with rising water temps, and never removed it.

Tank stock: 3 albino bristlenose pleco "fry," each around two inches. 4-5 pygmy cories I couldn't catch when I turned this into a BN growout tank, and one pugnacious female betta. Tank gets 2, sometimes 3, 45-50% water changes a week. Tank is fed every two days, mostly algae wafers, zucchini, lettuce, squash. Live bloodworms were fed two days ago.

The thing is that until today, I had -4- plecos. And did the water changes to keep up with the waste problems, the hornwort and various crypts to help with the rest. Tank lights are on roughly 13 to 14 hours a day.
