Crooked Dojo

My two year old dojo is very ill. Its body has gotten very bent as if it is broke in several places. He still eats but mostly lies on his side at the bottom of the tank. I have had the tank set up for two years and have never had a problem. In the past two weeks I have lost what appeared to be three prefectly healthy fish. I have checked my tank. PH 7.0, no nitrates or amonia. Any suggestions?
