Help with injured Sidthimunki!!!

I noticed this evening during feeding that one of my Chain loaches was just sitting in back corner. Upon closer inspection, its stomach looks swolen & there is a red(blood) spot where it looks like it has been pierced. Tank parameters are:

2 - Sparkling Gouramis
6 - Pristella Tetras
8 - Dwarf Pencilfish
4 - Neon Tetras
4 - Pygmy Corys
6 - Chain Loaches
Ammonia - 0; Nitrites - 0; Nitrates - ~15ppm; ph - 7.1

All other fish in tank are fine. My 1st question would be how to treat this kind of wound? Should I move to quarantine tank? Secondly, any ideas what could cause this?
