Plant Problems in my loach tank

My 55 Gallon tank that has 110 watts of light over it is a fully planted tank. The contains botia striata among other fish. The problem I have is in the last week or so most of the leaves of my radican sword and my java fern on one side of my tank have decided to start to rot. The sword just gets bad patches randomly thoughout the leaf ... the area does not have to be an area that the loaches poked a hole in. Any help to keep my plants alive for my loaches I would appreciate it.

PS. I checked all the water chemistry and temperature and everything is just fine.

I also use flourish once a week.
