Modesta wasted away - now what?

My 3" modesta that had disappeared soon after arrival in the tank 6 weeks ago just showed up again, grown longer, but skinny as a rail, just in time to die. I would like to treat the tank for whatever killed him so that my currently healthy loaches stay that way. My water is pH7.8, should I try the Levamisole hydrochloride anyway? I hate to monkey around with the tank pH - seems like asking for trouble.

Any ideas welcome. Thanks in advance for your help.
