When to euthanize

My almost one year old loach is suddenly very ill--he has ick, and I've been treating the tank at half-medication level for the past two days, but he is deteriorating. It looks like he is gasping for air, upright, and listless in the plants. I am thinking he is not going to make it, but I don't want to be pre-mature about euthanizing. Any thoughts on how to tell if the fish needs more time to recover or if the situation is too far gone?
