Sounds like it may be larval form of a freshwater clam. The free-swimming form embeds in the skin of the fish which then reacts to the presence of the larva by encapsulating it with cells that contain melanin. If you go fishing in freshwater lakes for bass, pike, pickeral etc. it's not unusual to find lots of these black spots on their skin and fins. The problem is self-limiting because eventually the larval form changes and pops off the fish. However, in an aquarium, the circumsatances are not conducive to further development.
